1st case of PAE with liquid embolic (Squid Peri) in Greece, in the same center, where the first PAE procedure for benign prostatic hyperplasia had been performed, 9 years ago.
-58 y old male patient
-Mild prostatic hyperplasia (PV=43ml, with relatively large intravesical prostatic protrusion on US), but with bothersome symptoms ( urgency, interrupted urination, nocturia). Baseline IPSS: 21
– Bilateral PAE with the unique Squid Peri 12cP lower viscosity version, ensuring distal access & excellent micro vessels infarction. Straightforward procedure and quick injection of the embolic, despite the small size of the prostatic arteries in this mildly enlarged prostate (Fig.1, DSA after embo of the right side and just before injection of Squid Peri in the left side, shows the tip of the microcatheter-arrow- in the left PA and radiopaque Squid-dotted arrows-in the right PA)
– Contrast-enhanced ultrasound (CEUS), performed in the angio-suite, immediately post PAE, indicated extensive infarction of both prostatic lobes (Fig. 2, ‘inf’ denotes the infarcted area of the prostate and arrow indicates the intravesical prostatic protrusion).
– Apart form a mild “post embolization syndrome” the procedure was well tolerated, with no complications. The patient was dismissed 8 hours post PAE.
– One month post PAE, unenhanced CT showed radiopaque “casts” of Squid Peri occupying the prostatic arteries and many of their intraprostatic branches bilaterally (Fig. 3, dotted arrows). An early symptomatic improvement was reported by the patient (IPSS: 14) and a degree of prostate shrinkage (PV=32ml) was noted on US.
